Reporting to the Program Manager, the Older Adults Outreach Worker, Aging in Community shall hold the responsibility to provide person-centered ‘system navigation’ and support services to older adults experiencing or at risk of experiencing psychological, mental health, or addictions.
The successful candidate will be kind, compassionate, organized and will provide accurate and timely information to the Program Manager. The successful candidate is to serve their clients predominantly through face-to face in-home and community visits. The successful candidate provides specific supports through the coordination of safe, timely, effective, and efficient supports, resources, and services to address their unique set of needs.
RESPONSIBILITIES AND ACCOUNTABILITIES
- Liaise with natural and professionals supports.
- Conduct comprehensive outreach services, assessments and evaluations in locations convenient to the client, such as face-to-face in-home visits, community or neighborhood settings.
- Develop a service plan and goals for clients.
- Access critical resources and tools to improve individual well-being and emotional support advocacy.
- Build and maintain the development of the collaborative service delivery and system navigation model with agencies participating in TWIN, including the Carya, Calgary Chinese Elderly Citizens Association, Calgary Seniors Resource Society and 211.
- Measure clients' progress regularly and adjust services accordingly.
- Build and maintain existing collaborative work with the health system (i.e. PCNs, home care) to establish effective referral pathways for older adults experiencing, or at risk of experiencing physiological, psychological, mental health or addictions issues.
- Work collaboratively with the Government of Alberta and other organizations as identified by Government throughout the project to identify, develop, and implement reporting and evaluation measures, which align with a Recovery-oriented Systems of Care framework.
- Assist with program development and implementation aligning with best practices.
- Increase client knowledge of community programs and support, in effort to reduce isolation.
- Carry out case management functions including convening case conferences, liaising with community services. and other professionals involved (especially education, social services, justice and health).
- Ensure the maintenance of accurate records in line with practice policy.
- Knows and adheres to the formal and informal departmental goals, standards and policies and procedures.
QUALIFICATIONS
- Degree/Diploma in Social Work as well as a registered member in good standing with their respective professional association/college.
- A minimum of 3 years working with individual/older adults in adverse situations.
- Applied Suicide Intervention, Non-Violent Crisis Intervention would be an asset.
- Must have effective interpersonal and communication skills and the ability to collaborate and build strong networks with a range of community stakeholders and service providers.
- Familiarity and aptitude to learn new software and information systems.
- Valid driver’s license, access to a reliable vehicle, satisfactory driver’s abstract.
- Current First Aid/CPR-AED and BLS- HCP.
- Mental Health First Aid or ASSIST.
- Satisfactory Police Criminal Background check including Vulnerable Sector Check is required.
