Mental Health, Team Lead

Calgary Rural Primary Care Network (CRPCN):

The Calgary Rural PCN works together with communities, family doctors and other health team members to do whatever we can to help you be as healthy as possible. We are working together to change the delivery of health care in our communities from reactive to preventative by providing services to patients that fit their needs through supports we have in our member clinics, such as nurses, dietitians, and other health care professionals. We collaborate to provide the tools and education to empower patients to work on their everyday health needs.

Position Summary:

The Mental Health Team Lead plays a critical leadership role within the CRPCN and is responsible for managing staff and supporting providers in delivering high quality care and services to patients within our communities.

The position leads the interdisciplinary care team and works with Clinic Managers, Executive Director, Medical Director, and other team members in the delivery of high-quality care within efficient and accessible PCN programs and services. The role also contributes to the design, development, delivery, and evaluation of clinical programs and services.

Key Responsibilities:

  • Managing a team of Behavioural Health Clinicians and provide leadership, oversight and support for daily operations
  • Support effective team-based care and patient medical home models by establishing clear roles and expectations, clinical workload and capacity management, managing service agreements for embedded staff, and resolving issues in a timely and effective manner.
  • Recruitment, onboarding, and performance management.
  • Create spaces for collaboration, best practice sharing, and development of clinical support tools (policies, standards of care, and standard operating procedures, etc.).
  • Work closely with member clinics and PCN Clinic Managers to ensure efficient and effective patient flow including access and referral management, cross-coverage, and adherence to practice, privacy, and occupational health and safety standards.
  • Contribute to planning, development, implementation, reporting, and evaluation of CRPCN programs and services.
  • Support cross-team collaboration, team building, ongoing learning opportunities, and alignment across geographically dispersed team of professionals.
  • Represent CRPCN in Zonal and provincial meetings.

 

Educational Requirements:

  • Masters Degree in Social Work or other health related field
  • Registered Clinical Social Worker preferred
  •  

 

Experience and Skills Required:

  • Previous experience managing or providing supervision and support to mental health therapists and/or counsellers
  • Performance management, supervisory skills, and experience is an asset.
  • Ability to problem solve, prioritize, and organize work effectively with minimum supervision.
  • Familiar with computer applications.
  • Effective communication and mentoring skills in a team environment.
  • Knowledge of clinical practice standards and regulations, team-based care, and patient medical home.

Working Conditions:

  • Multi-site, clinical office environment – some work from home
  • Travel to various CRPCN work sites and physician member clinics is required

Reports to Director, Clinical Operations
